F.A.C.T.S. Payments are taken out on the 5th and/or the 20th of each month, either out of checking
or savings accounts.  If checking account, must attach a voided check to the F.A.C.T.S. Form when
sending in.

Can make Bi-monthly payments, need to fill out two  F.A.C.T.S. Forms and attach a voided check to
each one if coming out of a checking account. Note:  There is a $38.00 fee for each F.A.C.T.S. Form.
